How to Use Templates in Supanote

Standardize Your Clinical Documentation with Professional Templates

Templates ensure consistent, compliant clinical documentation while reflecting your unique therapeutic approach. Supanote provides comprehensive built-in templates for all major documentation formats, plus the flexibility to create custom templates that match your specific clinical style and practice requirements.

  • Supports various therapeutic modalities and practice styles


1. Using Built-In Templates

When you create a new note (via Live Session, Dictation, Upload Audio, or Input Text), you’ll see a Template dropdown.


From there, you can choose:

  • SOAP → Subjective, Objective, Assessment, Plan (classic, detailed, insurance-friendly)

  • DAP → Data, Assessment, Plan (more concise than SOAP, still structured)

  • BIRP → Behavior, Intervention, Response, Plan (common in behavioral health)

  • GIRP → Goal, Intervention, Response, Plan

  • PIE → Problem, Intervention, Evaluation (very clinical, very direct)

  • EMDR → Pre-built for EMDR session documentation

  • Intake → First-time client sessions (demographics, history, presenting issue, impression)

  • Treatment Plan → Long-term goals, interventions, timelines, and measurable outcomes

💡 Tip: Don’t stress about which template to pick at the start - you can always switch later.

2. Switching Templates on an Existing Note

Already generated a note but want to see it in another format? Easy.

  1. Open your note (from Notes Pending Review).

  2. In the Note Preferences panel (left side), click Template.

  3. Choose a new format (SOAP, DAP, EMDR, etc).

  4. Supanote will instantly reformat the note into the selected style.

Note: Switching templates overwrites the current version. If you want to keep the old format, copy/download it first.

3. Creating Your Own Custom Template

Every therapist has their own rhythm. Maybe you always start with “Presenting Concerns,” or you want a “Homework” section. Supanote lets you build this into a reusable template.

Here’s how:

Go to the Templates tab

  • From your dashboard, click Templates (top menu)

  • you can view the list of standard templates

  • For each template you can preview an example by clicking on "preview"

  • You can also personalize the standard template by clicking on "Personalize" from the preview

2. Build or Edit a Template from Scratch

For deeper control, you can create your own reusable template:

  1. Go to Templates tab

    1. From your dashboard, click Templates in the top menu.

  2. Choose how to start

    1. Use existing note → Paste in a past note you like. Supanote will convert it into a reusable template.

    2. Start from scratch → Add sections one by one (SOAP, DAP, Intake, etc.).

  3. Add the sections you want

    1. Mix and match formats. Example: SOAP + Treatment Plan, or DAP + Risk Assessment.

    2. Use Custom Sections for your own headings (e.g., “Homework Given,” “Supervisor Comments”).

  4. Save your template

    1. Give it a clear name (e.g., “EMDR Progress Note,” “Short SOAP”).

    2. Click Save Template.

Your custom template is now available in the dropdown whenever you generate a new note.

4. Making Supanote Learn Your Style

Every time you edit a draft, Supanote gets smarter about your preferences.


Examples:

  • Always prefer “Pt” instead of “Client”? Save it under Note Preferences.

  • Use CBT interventions often? Add “cognitive restructuring, behavioral activation” to your default instructions.

  • Want a more concise voice? Tell Supa AI: “Summarize in bullet points, keep sentences under 12 words.”

Over time, your templates + edits + Supa AI instructions = notes that sound like you wrote them.

💡 Pro Tip: Create separate templates for different modalities (e.g., CBT, DBT, IFS) so Supanote instantly frames the note in the right clinical language.
